Note that there are 3 Participant Agreement forms (for parents of a minor, legal guardian, or adult); you need only fill out 1 of the 3 Participant Agreement forms based on whether the application is for a minor or an adult and whether the minor’s paperwork is being filled out by a parent or a legal guardian.
